Endometriosis - our passion

Dr Holland and his colleagues, Dr Gary Swift and Dr Abigail Evans, have brought together their extensive skills and experience in the treatment of endometriosis to form SHE Medical.

Endometriosis is a complex disease that has a tremendous impact upon the well-being and quality of life of the women that it affects. All of our clinicians at SHE Medical have a special interest and expertise in persistent pelvic pain and Endometriosis. They each have the specific skills needed to treat Endometriosis effectively.

Diagnosis relies on laparoscopy (surgical investigation with the intention of excision of active disease), but concurrent medical treatment is essential for this condition. This typically involves hormonal therapies such as the contraceptive pill or the Mirena IUD.

Treatment for endometriosis depends on a number of factors including:

  • the severity of symptoms,

  • the extent of the endometriosis,

  • the woman’s age and

  • her outcome requirements (e.g. reduction in pain, improved fertility).

The gold standard in surgery is excision of all visible disease (as opposed to ablation or simply burning deposits).  Restoration of normal internal anatomy, often distorted by endometriosis, is also a goal of surgery.  Additionally, a comprehensive approach to endometriosis assessment and management includes, but is not limited to:

  • concurrent surgical assessment for bladder pathology (bladder pain syndrome/interstitial cystitis);

  • assessment of pelvic floor function including:

    • pre-operative physiotherapy;​

    • consideration, where appropriate for adjunctive therapies such as pelvic floor Botox;

  • assessment and management of any associated vestibulodynia concerns​;

  • assessment and management of any chronic pain concerns.​

 

It is important to see a gynaecologist who is skilled and competent in the comprehensive surgical and non surgical management of endometriosis and its associated conditions.
